The attendee information form collects important information about the emergency contacts of play experience attendees.
It is essential for insurance purposes that the facilitator has emergency contact details for each attending family, therefore it is important that both the attendee information form and membership forms are completed.
It is the facilitators responsibility to keep the form safe and secure, in line with Play Matters Australia Privacy Policy.
